The Premier Concierge Medical began in Newport Beach as the Institute of Plant-Based Medicine, founded by Dr. Angie Sadeghi in 2020. The original conviction was straightforward: the gap between medical care and nutritional knowledge was the largest unaddressed lever in primary and specialty care, and consolidating specialists under one organization would let patients actually feel that integration in their care.
Six years on, the practice has grown into a multi-physician, multi-specialty institute serving members across Orange County — and the rebrand to Premier Concierge Medical clarifies what the practice has always done. Plant-based remains foundational to the medicine. Concierge access is the structure that makes the medicine deliverable.
Why concierge, and why now
Conventional primary care runs on volume. The economics force fifteen-minute visits and a calendar booked weeks in advance. That's not a failure of physicians — it's a failure of the structure they're working in. The medicine that prevents chronic disease, optimizes hormones, turns around metabolic dysfunction, or addresses the small things before they become the large things — that medicine takes time and continuity. Concierge membership is what makes that time and continuity possible.
We're explicit about it: members are paying for access and coordination, not for procedures or medications. The clinical work is the same standard of care any patient would receive. What changes is how the practice meets you — same-day or 24-hour appointments, direct messaging with your physician, no waiting room shared with the general medical schedule, and a primary care anchor who actually knows what your specialists are doing because they're the same building.
Why plant-based
The plant-based foundation is not an ideology, and members are not required to adopt a plant-based diet to belong here. What plant-based means in our practice is a clinical orientation: cardiovascular and metabolic outcomes are dramatically affected by dietary pattern, the evidence is unambiguous, and the practice is willing to integrate that evidence into how we actually treat patients rather than treat lifestyle as somebody else's problem.
Dr. Sadeghi is publicly aligned with Beyond Meat and is a public educator on plant-based gut health — that's a real expression of the practice's posture, not a marketing affectation. Cardiology consultations recommend dietary intervention alongside lipid management. Weight loss programs combine GLP-1 medication with registered-dietitian counseling. The plant-based identity is what lets us hold that line when conventional practice would default to "diet and exercise" advice with no follow-through.

That's the version we run. Dr. Sadeghi and Dr. Kalra carry the gastroenterology workload. Primary care, cardiology, mental health, dietetics, hormone optimization, weight loss, and aesthetics fit into the picture in concert with the GI service, not in parallel to it. The primary-care anchor is introduced to each member during the new-patient consultation rather than promoted as a named figure here.
