Concierge membership is a relationship-based care model that gives members direct phone access to their physician, same-day or 24-hour appointments, telemedicine and messaging five days a week, and integrated coordination across the practice's service lines.
How much does concierge membership cost?
Membership is offered in two monthly tiers: Wellness at $99 per month and Premium at $475 per month. Both include direct physician access, the office line, and coordinated care across the practice's service lines.
What is included with membership?
Member benefits include direct phone access to your physician, office line access, same-day or 24-hour appointments, telemedicine five days a week, messaging five days a week, free venipuncture for blood draws, a free B12 shot every three months, and 50 percent off available supplements.
Can I add family members to my plan?
Please contact the practice to discuss membership options for additional family members.
Do I have to be plant-based to be a member?
No. The plant-forward foundation describes how the practice delivers medicine, not how members are expected to live. Members come from every dietary background. Where the evidence supports a dietary intervention, the team will discuss it openly and offer resources to act on it, but the choice remains yours.
Does membership replace my insurance?
No. We are a hybrid practice, so membership works alongside insurance billing and cash-pay. Concierge membership fees are typically HSA and FSA eligible. For specific coverage questions, please call the office at (949) 404-4444.

Who are the gastroenterologists?
Gastroenterology is led by Dr. Angie Sadeghi, MD, Founder and CEO, and Dr. Amita Kalra, MD. Both are board-certified gastroenterologists. Services include colonoscopy, upper endoscopy (EGD), management of IBD and IBS, and plant-based gut health.
What does the gastroenterology service include?
Gastroenterology covers diagnostic colonoscopy, upper endoscopy (EGD), long-term management of conditions like inflammatory bowel disease and irritable bowel syndrome, and plant-forward gut health, with registered-dietitian support integrated into care.
Do you offer weight-loss support with GLP-1 medications?
Yes. The weight-loss service is physician-supervised and combines GLP-1 medications, plant-forward nutrition, and behavior support throughout the program.
What does the hormone replacement therapy program involve?
Hormone replacement therapy is led by Dr. Sadeghi and includes bio-identical HRT for women and TRT for men, with comprehensive baseline labs, 90-day and 180-day lab pulls, and follow-up visits at three-month and six-month checkpoints.
What structured clinical programs are available?
The practice runs four structured clinical programs: Lifestyle Reset, Hormone Optimization, Executive Cardiovascular Health, and Mental and Cognitive Wellness. The initial consultation for these programs runs 60 minutes, and the Executive Cardiovascular Health physical is one hour.
Is mental health care available?
Yes. The practice offers confidential psychotherapy and psychiatric care, integrated with the rest of a member's medical care.

How quickly can I be seen?
Members have access to same-day or 24-hour appointments, plus telemedicine visits five days a week. Premium members also have direct physician cell phone access.
Are telemedicine visits available?
Yes. Telemedicine visits are available five days a week, and secure messaging with the practice is available five days a week as well.
What are your office hours?
The office is open Monday through Thursday from 8:30 AM to 4:30 PM and Friday from 8:30 AM to 12:00 PM. After-hours messages are returned within one business day.
What should I do in a medical emergency?
For a life-threatening emergency, dial 911 or go to the nearest emergency room. For non-emergent concerns, call the office at (949) 404-4444 during office hours. Membership does not replace emergency services.
Can I keep my outside specialists?
Yes. Many members use the practice as their primary-care and gastroenterology home while continuing with outside specialists. The practice's coordination support extends to working with your outside specialists when relevant.
Where is the lab work sent?
Lab work is sent out to LabCorp and Quest based on patient insurance and clinical needs. There is no in-house lab, so a draw routes to the vendor most appropriate for your plan and the panel being run.

Yes. Members who pay cash are welcomed equally to members who use insurance. No program is cash-only and no program is insurance-only.
