Gastroenterology

Specialized GI care, rooted in lifestyle medicine.

Our gastroenterology service provides diagnostic and therapeutic GI care — colonoscopy, upper endoscopy, and long-term management of conditions like IBD and IBS — with dietary integration built in. It is for patients who want specialist gut care that takes nutrition seriously, and it is delivered through concierge membership — specialist visits and care coordination are covered by membership, while procedures such as colonoscopy and EGD are billed separately, where insurance applies.

Led by Dr. Sadeghi and Dr. Kalra. Diagnostic procedures, advanced therapeutics, long-term care of chronic conditions — and the dietary integration that makes the medicine work.

Diagnostic and procedural services

Procedures and care offered.

Screening · Diagnostic

Colonoscopy

The gold-standard screening procedure for colorectal cancer and the most reliable way to evaluate symptoms originating in the lower GI tract. Recommended starting at age 45 for average-risk patients, earlier with family history or symptoms.

  • Bowel preparation protocol provided in advance, with concierge-line support if questions arise during prep
  • Procedure performed under conscious sedation or monitored anesthesia per patient preference and clinical indication
  • Polyp removal during the procedure when indicated
  • Same-day results discussion; biopsy results within standard turnaround
  • Recovery support through the office line during the 24 hours post-procedure
Diagnostic · Upper GI

Upper Endoscopy (EGD)

Direct visualization of the esophagus, stomach, and duodenum. Indicated for persistent reflux unresponsive to medication, dysphagia, suspected ulcer disease, anemia evaluation, and screening for Barrett's esophagus in appropriate patients.

  • Diagnostic capability for reflux, hiatal hernia, gastritis, ulcers, Barrett's esophagus, celiac disease (with biopsy)
  • Therapeutic capability for dilation, hemostasis, polypectomy where indicated
  • Tissue biopsy for definitive pathology
  • Same-day results discussion
Chronic care

Inflammatory Bowel Disease (Crohn's, ulcerative colitis)

IBD is a chronic, lifelong condition that benefits enormously from continuity of care and the kind of access concierge membership provides. Flares need to be addressed quickly. Medication regimens evolve. Dietary support matters. The concierge model is structurally suited to this kind of long-term partnership.

  • Diagnostic workup (colonoscopy with biopsy, imaging coordination, lab monitoring)
  • Medication management — biologics, immunomodulators, aminosalicylates as appropriate
  • Plant-based dietary support — anti-inflammatory protocols developed with the registered dietitian
  • Coordinated specialist support when surgical or advanced therapy is indicated
  • office line access during flares
Functional GI

Irritable Bowel Syndrome (IBS)

IBS is the most common functional GI condition and the one most poorly served by conventional volume-based primary care. Successful management requires real time spent on dietary triggers, stress integration, sleep, and the slow iteration of an individualized plan. The concierge model is built for that work.

  • Comprehensive diagnostic evaluation to rule out structural disease
  • Plant-based and FODMAP-aware dietary intervention coordinated with registered dietitian
  • Stress and lifestyle integration with mental health support when appropriate
  • Targeted pharmacotherapy when indicated

Plant-based gut health

Dr. Sadeghi's clinical and public-education work centers on the relationship between dietary pattern and gastrointestinal health. The microbiome is shaped by what patients eat. The evidence on fiber, fermented foods, and predominantly plant-based dietary patterns for gut health is increasingly clear, even as the field continues to evolve.

For members, this means GI care is not separated from nutrition. Dietitian consultations are integrated. Plant-based protocols are offered when clinically appropriate. The medicine is not gastroenterology with a wellness sticker — it is gastroenterology that takes nutrition seriously enough to act on it.
